The MMSC Subject Dossier Initial Record Pack turns the ordinary business of living with a cat into something considerably more organised.
Designed as the foundation of the wider MMSC Subject Dossier system, the Initial Record Pack combines the procedural character of an official case file with records that are genuinely useful to keep. Identification details, microchip information, vaccination history, medication, veterinary visits, feeding, weight, insurance, behavioural observations and household information all have somewhere to live — without reducing the dossier to paperwork for paperwork's sake.
The first month is documented progressively. Start with the subject's arrival and first 24 hours, review how things are developing after seven days, identify emerging patterns at fourteen days, and complete a thirty-day assessment before transferring the subject into the ongoing record.
After that, the dossier remains useful. Long-term medical, vaccination, preventative treatment, weight, nutrition and behaviour records can continue to be updated for years.
The pack contains 22 A4 record sheets, including:
- File Control & Document Register; Subject Identification; Microchip & Registration; Origin & Transfer; Household Occupancy; 24-Hour Arrival Observation; Seven-Day Review; Fourteen-Day Pattern Assessment; Thirty-Day Subject Assessment; Ongoing Record & Intake Summary; Veterinary & Emergency Contacts; Vaccination; Preventative Treatment; Medical Procedures & Surgical History; Medical History & Known Conditions; Veterinary Visits; Medication; Feeding, Nutrition & Hydration; Weight & Body Condition; Insurance; Behaviour & Handling; Territory & Positioning; and Incident Report.
Digital Edition
The Digital Edition provides the complete Initial Record Pack as an A4 PDF.
Print the records you need, add replacement or continuation copies when required, and build the dossier at your own pace. It is particularly suited to multi-cat households, long-term record keeping, or survivors who want the flexibility to reprint individual forms whenever they are filled, damaged or updated.
Your original file remains available as the clean master for your own household use.
Physical Edition
The Physical Edition arrives as a complete A4 loose-leaf record set, printed in-house and supplied together in its document envelope.
There is no binding deliberately restricting how the dossier develops. Records can be removed for completion, reordered where needed and supplemented later with continuation sheets and additional Subject Dossier modules.
It is the version for survivors who want to open the envelope, begin the file and immediately start documenting the subject without printing anything themselves.
Built to become a larger record
The Initial Record Pack is only the beginning of the MMSC Subject Dossier System.
Future continuation and specialist modules can extend the same dossier across Health & Welfare, Behaviour & Observation, Incident & Evidence, Annual Review and other areas without replacing the original intake record.
The first thirty days establish the file.
Everything after that becomes evidence.
Please note: The MMSC Subject Dossier is an organisational record-keeping system for cat owners. It does not replace veterinary records, professional veterinary advice or other official documentation.
